Amethyst Center, Inc. provides services in the comfort of the individuals home..
Services are provided to adults in need of mental , physical and rehabilitative supports ages 18 through end of life. In addition Amethyst Center provides housing services to Homeless individuals, Elder Care, Respite, Hospice, Companion services and Health and Wellness services.
Our staff are fully certified and trained to assist individuals and meet their needs in a dignified humane manner.

Amethyst Center is equipped to serve and meet the needs of each individual. A special service design is created at your request, to ensure quality service and care.
The above services are not limited and other requests may be made. Our Service Agreement details all terms and conditions for services. Fees will be based on assistance needed.
The base fee for services is: 18.00-20.00per hour, Monday-Friday 12:00AM to 11:59PM.
Weekend and Holiday rates are: 24.00per hour. Weekend is considered Saturday 12:00AM to Sunday 11:59PM.
Private insurances such as Long Term Care, Short Term Disability, Veterans benefits, provide assistance with payment for staffing services. (If Eligible) . Please see below a list of other accepted insurances.
Who is Eligible?

Individuals ages 18 through end of life in need of community supports due to a disability, injury, mental or physical challenges.

Amethyst Center, Inc. has a full service team of individuals ready to assist you with your questions and concerns.

SPECIAL PLANS AND SERVICES:
All customers receive a free consultation which includes: explanation of services, availability and cost of services.
PLAN A: COMPLETION OF FORMS:
1. Consultation: No charge
2. Application and Completion of Forms: 300.00 this is a onetime cost.
3. Food Benefits
4. Medicaid
5. Home Chore services
Additional Forms: 300.00
PLAN B: MONTHLY STAFF: 300.00
1. 4.5 Hours of staffing per week at 18.00 per hour
2. Additional staffing hours billed at 18.00 per hour
3. Completion of Forms: no charge
4. Forms to be completed (same as above) in Plan A
5. Six (6) month contract required
6. Advance Payment required for service
7. Billing is done in Advance and staffing will not commence until full payment is received.

PLAN C: STAFF AND TRANSPORTATION: 300.00 PER MONTH:
1. 4.5 hours per week at 18.00 per hour
2. Transportation services at 1.60per mile
3. Completion of Forms (Same as in Plan A and B)
4. Six months contract required
5. Billing is done in advance and services will not commence until full payment is received.

PLAN D: TRANSPORTATION SERVICES: 60.00 ROUND TRIP AND 3.00 PER MILE:
1. 24 hour notice required
2. Non-Medical
3. Wheelchairs vehicles available
4. Fee must be paid upon arrival

PLAN E: GUARDIANSHIP SERVICES: 20.00 PER HOUR:
1. Customer may elect Amethyst Center, Inc. as Guardian at no charge.
2. Customer may hire Amethyst Center, Inc. for monthly visitation and reporting.
3. Other services as designated by Legal Guardian.
4. Fee to be paid by Guardian

VISITATION AND COMPANION SERVICES:
These services are provided at the rate of 20.00 per hour.
Customer may hire Amethyst Center to visit a loved one in the Nursing home, assisted living facility or in their own home when you are unable to. Amethyst Center will provide written and verbal reports directly to you regarding the health and wellness status of your loved one.
If you need someone to be a companion to your loved one or just take them to the leisure time activities they like and spend time with them to make sure they are not lonely or unattended to.
Let us be your personal assistant when time is of the essence and you simply cannot make it. We can fill that space for your and ease your comfort zone.

ADDITIONAL SERVICES AVAILABLE: 148.00 (1) DAY OF SERVICE:
· Minimum of 4 hours required
· Maximum of 8 hours
· Additional hours will be billed at 18.00 per hour Monday-Friday and 24.00 per hour Saturday-Sunday.
RESPITE SERVICES:
At your request these services are billed at 18.00 per hour. There special discounts available for seven days or more.

****FEES FOR ALL SERVICES MUST BE PAID IN ADVANCE. AMETHYST CENTER, INC. SERVICES WILL NOT COMMENCE UNTIL PAYMENT IS RECEIVED.
ALL CONSULTATIONS ARE AT NO CHARGE. PLANS B AND C ARE DISCOUNTED FOR MONTHLY SERVICE. ALL OTHER SERVICES ARE BILLED AS NOTED.
